Your data
Data protection and consent
Before your first call you complete a short consent form. Everything on it is optional. Ticking nothing does not change your care, and you can change your mind at any time by emailing [email protected].
Separately, we hold and use your health records in order to provide your care. This is explained in our Privacy Policy and is not a matter of consent. Ultimate Health is registered with the Information Commissioner's Office as a data controller, and your health information is handled as UK GDPR requires.
What the consent form asks
Your GP, where it would help
A short letter setting out what we found and what we suggest. Most clients say yes, because it keeps everyone working from the same information.
Other practitioners you have named
Only the people you tell us about, and only what is relevant.
Your name and contact details shared with the testing laboratory
We need this to order a test for you, because the laboratory posts your kit and returns your result. Some of the best laboratories are outside the UK and EU, including in the United States. If you would rather not, tell us and we will use a UK or EU laboratory where an equivalent test exists, though the choice is narrower.
Occasional emails
What we are learning, what is new, and the odd offer. Never often, never sold on, and every email has an unsubscribe link.
Your case discussed and written about, anonymously
Clinical supervision, professional forums, teaching and writing. Your name, address and contact details are never shared, and nothing that could identify you is used.
Your calls recorded
With your permission, calls are recorded so that nothing you tell us is missed and your notes are accurate. Recordings are kept securely as part of your client record and are never shared. You can ask for any call not to be recorded.
Your records
Client records are kept securely for eight years from the date of the last entry, then securely destroyed. You can ask to see your records, in line with your legal rights.
Changing your mind
You can withdraw any consent at any time by emailing [email protected].
